I'm booking a Christmas return flight from Guam and ran into an issue. The outbound from the US East Coast can be kept entirely within the US, but on the return, if I fly GUM-NRT-EWR, do I still need to go through CBP? Without a valid visa, is GUM-HNL-EWR my only option?

Don't risk transiting through Japan without a valid visa—check with your airline first to see if they'll even let you board.

It doesn't have to be just the GUM-HNL-EWR route—the rules for departing from and returning to the U.S. East Coast may differ, and they can vary significantly depending on whether you hold a green card, a U.S. passport, or a different visa type. It's best to verify separately based on your status.

Is the GUM-NRT-EWR itinerary you found on a single ticket or separately purchased? This affects whether your baggage is checked through and whether you need to enter Japan during the connection.
For a routing like Guam → Japan → New York, check-in and the gate may ask about Japan transit or entry eligibility—don't just focus on the CBP part.
