Question about installment payment plans for medical bills on MyChart

I've been in the US for many years and this is the first time a somewhat large bill has shown up in MyChart. It turns out they offer a 4-installment plan, and the total of the four payments is exactly equal to the original amount owed on the bill—in other words, it's a 0% APR plan. I know medical bills often go unpaid, and I can understand why they'd make an offer like this, but it still seems too good to be true. It looks like there's no cost at all—is there some kind of catch, or is it actually a loophole? For example, if I set up a 4-installment interest-free plan every time I get a bill, it seems like I'd only come out ahead with no downside. Are there any risks or problems with that?

I chatted with various LLMs, and they basically all said hospitals proactively offer interest-free installments because they're afraid they won't collect the money. Some LLMs did point out that this kind of offer might come from the hospital itself, or it might be outsourced to another financial company, and it's hard to say how those financial companies handle things (like whether they check your credit). Some also reminded me to watch for hidden fees like administrative fees, but I don't think I've seen anything like that (they surely wouldn't fail to disclose it upfront and then suddenly charge my credit card, right?).

I'd like to ask the forum experts: have any of you tried installment plans for medical bills, and are there any pitfalls? Thanks!

Zero interest does save you money, but spreading it over four installments drags out the payment period. If you change jobs or move in the meantime and miss an installment, the late fees and credit impact end up being more of a hassle.

It varies a lot from hospital to hospital. Some set up payment plans directly through their internal billing system, with no credit check and nothing reported to the credit bureaus; others hand it off to an outside lender or billing management company, and the terms may not be the same. Best to first confirm which organization's name you're seeing in MyChart.
